Do I need to work for the government?

The short answer is maybe.


According to Malaysia’s Medical Act of 1971, every medical practitioner must serve a continuous period of no less than two years within the public sector upon being granted full registration before they can seek employment in the private sector. This is based on the latest guidance in June 2022.


If you wish not to work in Malaysia’s public sector, there are 3 criteria's for total exemption:


Criteria 1

Practitioners who are 40 years old and above at the time of application, with or without a postgraduate qualification

Criteria 2

Practitioner with a postgraduate qualification registrable with the National Specialist Register AND has secured an employment post in a public or private higher education institution in Malaysia as a lecturer in a medical faculty for a continuous period of not less than two years.

Criteria 3

Practitioner who is fully registered but did not secure any offer of employment in the public sector after six months of application.




How do I apply for exemption?

There are several things you are expected to have in hand to apply for exemption from government service, including:


For applicants over 40 years old


1) Copy of evidence of full registration with MMC

2) Copy of the offer letter

3) Copy of the resignation letter from the previous institution

4) Copy of the completed application form (if you submit a paper form)

For those who are fully registered but have not received any offer from the government service after 6 months of application.


1) Original copy of the government exemption application form

2) Copy of full registration with MMC

3) Evidence of application in the public sector

4) Letter of good standing from abroad

You may apply for the exemption by filling in the online form and ensuring that you have completed the checklists attached to the form. The form and documents may be sent in person, by post, or by email (scanned copies) to:

The application will be processed in 6-8 weeks from the time of receipt of all forms and supporting documents.


The approval letter/result will be sent to the applicant via post and your registered email address.

  • In most cases, all newly registered medical practitioners will be given a notice for compulsory service. This notice will be sent via post or sent over via email. Application for the exemption from government service may be made following this.
